Why a man can trigger a pregnancy test

The test detects hCG, full stop — the antibody sandwich doesn't ask who supplied the sample. Healthy men carry near-zero hCG (typically <2 mIU/ml, far under any strip's threshold). A genuine colored line in a man means something is producing hCG, and the notable producer is germ cell tumors — including some testicular cancers — which secrete hCG as a tumor marker. Clinicians literally use blood hCG in diagnosing and monitoring these cancers.

That's why the joke test earned a real place in public-health lore: cases exist where a gag positive led to an early tumor diagnosis. Early-caught testicular cancer has excellent cure rates; hCG made some of those catches.

Most work-ups find benign explanations; the visit is cheap either way.
