Late Period, Negative Test, Not Pregnant: 10 Other Causes of a Missing Period
When tests keep saying no but the period stays missing, something else is holding the cycle. The ten common non-pregnancy causes, what each looks like, and which need a clinician.

Repeated negative tests with a stubbornly missing period eventually flips the question from "am I pregnant?" to "where's my period?" — a question with its own well-mapped answer list. The ten usual suspects
- Stress. Cortisol suppresses the hypothalamic signals that trigger ovulation; a brutal month can delay or skip a cycle entirely. The period returns when the nervous system does.
- Illness — including routine infections. The body postpones reproduction while fighting; even a bad flu near ovulation time can push everything back weeks.
- Travel and shift work. Circadian disruption is an underrated cycle-scrambler.
- Weight change and under-fueling. Rapid loss, low body fat or intense training (alone or combined) can pause cycles — hypothalamic amenorrhea. Common in athletes; reversible with fueling.
- PCOS. Irregular, long or absent cycles with possible acne/hair changes — the most common hormonal cause; see PCOS and testing.
- Thyroid disorders. Both over- and under-active thyroids derail cycles; fatigue, temperature intolerance and heart-rate changes are companion clues. A simple blood panel checks it.
- Elevated prolactin. Can pause ovulation (occasionally with unexpected milkiness); medication-related or pituitary causes — clinician territory.
- Perimenopause. From the early-to-mid 40s (sometimes earlier), skipped and shifted cycles are the opening act.
- Medication changes. Starting/stopping hormonal contraception (post-pill fog), some antipsychotics, chemotherapy and others.
- Breastfeeding. Prolactin again — nursing suppresses cycles long after birth.
What to do while you wait
Keep a simple log (dates, tests, symptoms, life events) — it's the most useful artifact you can bring to an appointment. Retest weekly with morning urine while the period is missing; ovulation can return without notice, and conception can precede the first period back. When it stops being a waiting game
- Three months without a period (and not pregnant): book regardless of cause candidates.
